To make a stencil, first, you come up with a design, usually a simple black and white one. In the design, there sould be no "islands", free-floating bits of white- they should all be connected.

You trace that design onto a piece of acetate, and carefully cut it out with an Exacto-knife.

To paint the design onto a surface, tape the stencil down flat or use a spray adhesive, and you can either use spray paint or a paint roller to apply paint. Spray in good for a quick job on a flat, even surface. A paint roller is good for if you want to put the design on clothing.

Screenprinting is basically the same thing, except that there can be "islands", so your design can be more intricate.
